We know that not every patient has dental insurance, that's why we are proud to offer our Bliss Dental Membership Plan! For a low monthly fee, you will get all your preventive care, as well as discounts on additional treatment. And with a plan for adults, children, and periodontal patients, we can keep your whole family smiling.

The Bliss Dental Membership Plan is an annual savings plan for families and individuals that allow all members to receive premium dental services at greatly reduced prices. Unlike conventional insurance plans, with the Bliss Dental Membership Plan, there are no deductibles, no annual spending limits, and no waiting periods to begin treatment. The Bliss Dental Membership Plan benefits coverage begins immediately upon plan enrollment. Bliss Dental Membership Plan is a discount dental program, not dental insurance. This program is offered to our patients who do not have dental coverage.

  • Keep in mind that our membership plan is not insurance and is not a qualified health plan under the Affordable Care Act. Our membership plan includes a 30-day money back guarantee. You can receive a full refund up to 30 days after purchase if no benefits have been used. Payment for any treatment(s) not included in the membership plan is due at the time of service. Benefits may not be combined with insurance, other offers, or discounts. CareCredit cannot be used to pay for the membership plan. Our membership plan does not cover procedures completed by specialists outside our practice.
  • Dental services not provided by our practice are not covered by this plan.
  • Any dental procedures in progress or performed before or after a member’s eligibility period are excluded.
  • Any dental services provided to the member by the state, county, or municipal agencies, or dental services provided without cost to the member are excluded.
  • Dental plan discounts for dental services provided in association with benefits received from an alternate source (i.e. Worker’s Compensation) are excluded.
  • Our plan is offered to patients who do not have dental insurance coverage.

When can I start using my membership?

Your membership is effective immediately!

How do I schedule my preventive care?

Simply call our office and set up an appointment for yourself or your family members.

How can I use my Member Exclusive discount for restorative, elective or cosmetic treatment?

Simply schedule an appointment with our office and be sure to tell us you’re a member. When you arrive, we will make sure your membership is “current” and you will be eligible for instant savings off our regular fees.

How do I know if I need Perio membership?

If you are a current patient, please contact us to confirm you are on Perio Maintenance. If you are a new patient, we will make a recommendation after your initial exam.

What happens if I miss an appointment?

It will be your responsibility to reschedule. Please see our office policy regarding no-shows and appointment cancellation policy.

Can I add members to my account?

Yes! Your entire family can join.

Can I transfer my membership to another dentist?

Your Bliss Dental Membership Plan is solely with our office and as such is non-transferrable. If you move to another area, you may cancel your membership. Cancellation request should be sent in written 30 days prior to the expected date of cancellation.

If I cancel, can I get a refund on my membership?

No. Refunds will not be provided for dues paid under any circumstances, including failure to schedule and maintain appointments.

How much do I pay to sign up for my Bliss Dental Membership Plan?

You will pay only the Lifetime Activation fee to join.

Can I prepay my membership fees?

Bliss Dental Membership Plan are currently month-to-month only.

What information is required for me to sign up for my club membership?

An email address for receipts, credit or debit card, full name, address, phone and date of birth.

Legal & Disclaimers

Bliss Dental Membership Plan are not insurance but a payment arrangement provided by bliss dental arts for services rendered. Memberships are provided exclusively to uninsured patients of our practice and shall not be considered pre-payment for future services or payment for access to discounted services. Rather, our patient members are electing to make regular monthly subscription payments for preventive care instead of paying at the time of each service. Payment for any cosmetic or restorative treatment is due at the time of service. Membership dues must be current in order to receive services. A lapse of 28 days in dues will require a reactivation fee of six months’ dues. Bliss Dental Membership discounts may not be combined with any other offers or discounts. It is solely the patient’s responsibility to schedule and keep their appointments. No refunds will be provided for dues paid under any circumstances, including failure to schedule and maintain appointments. A patient member’s monthly agreement is with the legal entity of Shervin Moshashaei DDS, MBA Inc. Monthly dues and fees for dental services may change at any time. Payment management services are provided by ADIT.
